Effectiveness and safety of biological therapy in inflammatory rheumatic diseases

This thesis focuses on evaluating the effectiveness and safety of biological/targeted treatment in chronic inflammatory rheumatic diseases based on data from the ATTRA registry. The introductory chapters of the thesis give an overview of three rheumatic diseases - rheumatoid arthritis (RA), psoriatic arthritis (PsA) and axial spondyloarthritis (axSpA), characterising clinical manifestation, diagnosis, therapeutical options and current treatment guidelines. The work also contains a brief summary of information about planning, creating and maintaining a clinical registry and characterises specifics related to the analysis of registry data. The practical part of the thesis was aimed at two research questions. Recently, a treat-to-target (T2T) strategy was established for RA, PsA and axSpA. Studies from daily clinical practice concerning the advantage of following T2T over usual care are still lacking. Thus, the first goal of the thesis was to evaluate whether following a treat to target strategy after not reaching low disease activity within the first six months leads to a higher chance of meeting the treatment target at the twelve-month visit. Title: Guest dimensions of health status in patients with spondylarthritis ankylopoetica using the SF-36th Objectives: The major aim of the diploma thesis was the study and the evaluation of the quality of life of patients with the spondylarthritis ankylopoetica in the Czech Republic. Methods: The own research was conducted using the survey method by means of the standardized questionnaire Short Form 36 (SF-36), which deals with the Health Related Quality of Life . The Czech version of the SF-36 survey translated by MUDr. Petr, Ph.D. (2000) was applied. In order to reach the main aim of the thesis the research was conducted in a group of patients with spondylarthritis ankylopoetica (n=101) and among the same number of the general Czech population members. The achieved results were divided into particular age categories in the both groups. The statistically significant differences between the two analysed samples were subsequently verified. Results: The mean age of the patients with spondylarthritis ankylopoetica was 51,6±14,9 years, whereas it reached 52,3±15,5 years in the sample of the general population. Quality of life of patients after intensive care

Aims: The aim of the bachelor thesis is to assess the change in quality of life of patients and their close relatives after hospitalization in the ICU and assess the dependence of this change on patients' age, length of hospitalization in the ICU, length of artificial lung ventilation (UPV) and time since discharge from the ICU. Methods: The research was performed at the Anesthesiology and Resuscitation Clinic of the University Hospital at St. Anny in Brno. Quality of life was measured using quality of life questionnaires, in SF-36 patients, in EQ-5D-3L relatives. Age, sex, length of UPV and hospitalization and time since discharge from the ICU were monitored. The Wilcoxon test was used for statistical comparison of groups, the dependence of quality of life dimensions on other parameters was evaluated using Spearman's correlation. Results: The study group consisted of 61 patients and 45 relatives. With the exception of the physical pain dimension, the quality of life of patients after hospitalization was significantly lower than before hospitalization in all dimensions of the SF-36 questionnaires. Comparison between flow in the left internal mammary artery harvested via sternotomy and via a left anterior small thoracotomy with clinical impact for the patient Introduction: In the treatment of coronary artery disease (CAD), in addition to conservative therapy and percutaneous interventions, is irreplacebly belong coronary artery bypass grafting (CABG). The gold standard in the surgical treatment of CAD is CABG from a median sternotomy access using cardiopulmonary bypass (on-pump CABG). A less invasive and economically advantageous is CABG from a median sternotomy access without cardiopulmonary bypass (off-pump CABG) and last but not least CABG through a left anterior small thoracotomy (LAST) approach (MIDCAB - "minimally invasive direct coronary artery bypass"). Although the final technical implementation anastomoses is essentially identical in off-pump CABG and MIDCAB, the difference is very important during left internal mammary artery (LIMA) harvesting, especially through a LAST approach, which can be difficult for the surgeon. Cardiac care for the elderly - health and social aspects

Cardivascular diseases are still the major cause of mortality and morbidity in developped countries. From the available demographic datas ageing of population connected with increased number of patients is evident. These cardiovascular patients are treated conservative, interventional and surgical way. In the theoretical part of this work characteristics and patophysiology of ageing is described regarding particular body systems. Then there are mentioned individual diseases of heart which are currently being treated in a surgical way. It mainly deals with ischemic heart disease, disease of aortic, mitral and tricuspid valve and supraventricular arrythmia. There is also a desrciption of pre-operative preparation, course of the surgery and early postoperative period as well as pharmacological and interventionist therapy. Attention is paid to the differences in surgical care about seniors ? determination of operation risk, pre-operative course, operative course and postoperative rehabilitation. Quality of life is also mentioned as the important aspect while evaluating the success rate of treatment of the particular disease. The work gives information about its definition, possibilities of its observing and evaluation during the postoperative period. Last but not least social issues of senior patients are mentioned in the work as well. The research part is focused on the evaluation of the patients´ state during the pre-operative, perioperative and postoperative period. Particular associated diseases, data from the surgery and postoperative complications are also recorded. The research part then deals with the confirmation of individual hypothesis which were performed through the questionnaires before and one year after the surgery. It was answered by 310 patients.
Effect of Spa Therapy on Quality of Life and Consumption of Drugs in Patients Aged 35-50 Years with Musculoskeletal Disease.

This bachelor thesis titled ?The Effects of Spa Treatment on the Quality of Life and the Consumption of Drugs in Patients Aged 35 to 50 Years with Musculoskeletal Disease? deals, in the first chapters of the theoretical part, with the history and current status of balneology, individual spa treatments, and the importance of spa treatments in treating musculoskeletal diseases. The other chapters deal with health, the quality of life, its measurement and evaluation. Within the thesis, two goals were set: (a) to find out the effects of spa treatment on the quality of life and (b) to monitor the effects of spa treatment on the consumption of drugs in patients aged 35 to 50 years with musculoskeletal disease who were clients of Bertiny lázně in Třeboň. For fulfil the objectives of the thesis, the method interviewing was applied using standardized questionnaire SF-36 about the health related quality of life, which is suitable for finding out about the influence of spa therapy on the quality of life and health state of spa treatment clients. The respondents were examined prior their spa treatment, i.e. immediately after coming to Bertiny lázně in Třeboň, and then 3 months after the spa treatment, using the method of ?postal survey?. The results of this thesis show that the health state of patients with musculoskeletal disease improved after completing spa treatment, most distinctly in the domain of pain relief (30%) and in the vitality domain (26%). The consumption of drugs saw only minimal changes. Based on the results obtained, we can clearly say that spa treatment contributes to the improvement of the quality of life of patients with musculoskeletal disease and it also clearly points to the effectiveness of finance invested in balneology.
Contribution of Minimally Invasive Total Hip Replacement - MIS-AL, Health and Social Aspects of MIS-AL Approach

I want to present a prospective evaluation of one-year results in patients undergoing total hip arthroplasty from a minimally invasive antero-lateral approach (THA MIS-AL). And I want also to compare, by means of biochemical markers, the operative invasiveness of standard total hip replacement with that of the minimally invasive anterolateral (MIS-AL) approach.At one year after surgery we evaluated the quality of life by 60 patients using SF-36 questionaire and we compared the results with standard anterolateral approach and regional standard.
